What to do if you are injured
- Promptly notify your supervisor or employer and fill out an accident notice.
- Select your own Doctor of Chiropractic.
- Have your employer give you a copy of the accident report filed and take it with you to your Doctor.
- All reasonable and necessary examinations, x-rays and chiropractic care will be paid for by your employer's insurance company.
- Call (480) 595-6100 for an appointment with Dr. Tanner today.
- Please note, when an injury occurs, an employer has the right to have an injured worker seen by a doctor of the employers choice 1 time. (If you return to that physician a 2nd time, that physician would become your attending physician). After 1 visit to the employers designated physician you may report to a physician of your choice.
- If you have been to another physician and you are not getting relief or you have been to a different type of physician (Medical, Osteopathic, etc...) and you wish to change your treatment to Chiropractic, contact the Industrial Commission of Arizona at (602) 542-4661 and tell them you wish to see a new Physician. You can give the following information and then begin your treatment with us:
